Delivery notice (bon de livraison) generation

See #9 (closed) for when the delivery notice should be generated.

This issue is about what the document should contain.

Please provide the exhaustive list of information that needs to be on this document. If some information depends on the GRC and/or the accession holder (postal address, phone number, whatever), please also specify it, because we need to enrich the persistent model with this information.

An example document would be nice to have. Given that this will, I guess, need to be printed, please say if the format should be other than A4.
